In addition to a Client Care Coordinator, SARA has an exceptional RN Director of Patient Services (DPS) and a wonderful team of nurses (RNs and LPNs) who will review the Doctor’s Plan of Care, assess the patient’s condition, observe changes in health, make recommendations, and visit as needed.

Before SARA starts a new case, a Registered Nurse assesses the patient’s health to determine the level of care needed. The Registered Nurse reviews the Doctor’s Plan of Care, she speaks with family members and visits the patient. The Registered Nurse discusses both the physical and/or mental health needs of each patient.

The Registered Nurse meets with the caregiver to make sure she or he understands all the instructions needed to provide the best care possible in the home. If there is special equipment that must be used for the case, the Registered Nurse observes the caregiver’s ability to make sure that the aide will properly execute care instructions.

On an ongoing basis, the Registered Nurse Supervisor is informed of any changes in the health of the patient, and after conferring with the patient’s doctor, makes any adjustments needed to ensure the health of the patient. The aide is instructed to inform or report any issues or problems immediately to the nurse regarding her/his patient. Additionally, a nurse will make follow up visits to check on the patient’s health.

Our DPS, along with SARA’s RN Instructor of our HHA Training Center, are instrumental in providing and updating the skills of all our our caregivers.

Our team of nurses, RNs and LPNs, are also available for private care when a patient needs medical home care on a more extensive basis. Arrangements can be made with you Client Care Coordinator and SARA’s DPS when more nursing care is needed.

The SARA nursing team along with your highly trained Home Health Aide will ensure that you or your loved one are well taken care of at home.
